Q:
Will this come with everything in the box that originally comes with the Atomos Ninja V plus the 500GB SSD or just the Atomos Ninja V device minus original packaging plus the 500 GB SSD? Does the Atomos Ninja V also support the oversampled 4k HQ for the Canon R5 as well as in 60fps for it? Is this SSD fast enough as well to support? Sorry for a lot of questions! Very interested though!

Yes, the Ninja V is new in the box and includes all the standard accessories and packaging. The 500GB SSD is new and ships with the Ninja V.
I'll check with the Canon and Atomos reps about the frame rate question.
